Inserts came out?

Kinda crazy I had the inserts come out of a board the other day they whole top sheet spilt also its been a long time sence this has happened. Anyone else had this happen?

I've had inserts rip out of 2 of my old Hyperlite boards. They sent me the current year replacements within two weeks both times.
