Distance from left fender to tires is less than 1/2" so I pulled tires and pulled off I guess you'd call it a spindle ( Don't know ) then I turned them down so that when I put back on tires they would match the other side ( about 2" of clearance ) the problem is the left front tire keeps sliding and it's coming from inside the axel. How do I fix or does it need to go back to Extreme?

You may need to file material from between the split to allow you to tighten it more. It's splined on the axle so you may have worn the splines off the arm and a triangular file might help put them back a bit deeper.
Those are just guesses, It's not a problem I've seen first hand.

It's not the splines, I marked them and when trailer tire starts slipping down I pulled tire and my marks are in same location. It's coming from inside the axel but I don't know how that part works

Isnt the two "spindles" connected and use eachother as a type of stabilizer? Maybe it snapped in the middle..hmm.. maybe not than the other side would spin too. Not sure whats really going on but it sounds funny.

I would call Extreme..sounds like there may be a problem with the rubber bushing (not sure if that is the right term) inside of the axle.
