Looks like a wakeboard.

What is the board called and what kind of fins does it use?

Wow 6 fins! at least... looks like a lot of different configurations with that new board.

prototype folks.... i agree with duffy, very wakeboard in outline and characteristics with relatively straight nose, and slightly edge flared tail, straight nose and tail channels..... it has been done a few times before with limited success by some larger brands and a lot of smaller brands. However, none of the similar past designs ever had a world champion riding it or promoting it. fins are just fcs plugs arranged in various locations to test different fin set ups with the board. Off hand the fins look like the toe-in was reduced more like a wake board and less like a surfboard. I would suspect the final product will not have canard fins (generally termed 'twinzer fins'). Chase was pretty dead set against using canards, and preferred a more conventional quad set-up over anything else.
Looks like a fun board.
