Do you keep the boat parallel to the boat (like when you start with the board on the surface of the water) or do you hold the board pointed at the boat when starting with the board underwater?

Parallel.. same principles apply as to how you would normally get up.

yup, keep your weight on your heels is big. In the beginning when you start to do this all the time, you will want to kick the board a little in front of your body while standing up under water, especially when you are ready to be pulled out.

Keep the handle down low, under water, so you get pulled from teh center of gravity. I usually have mine just a touch above my center, and I lean back slightly just as the rope goes tight. Way easier than a surface deep water start once you get the hang of it.
